GUIDELINES ON USING THE DIVER MEDICAL FORM:
If you answer “NO” to ALL of the questions on the participant questionnaire, then you are on your way to pursue your dive training!
A positive response to a question does not necessarily disqualify you from diving. It only indicates that you must seek the advice of your physician prior to engaging in dive activities. You are required to obtain a physician’s release before being allowed to participate in the confined water sessions of your course. This is for your safety. If you answered YES to questions 3, 5 or 10 above OR to any of the questions on page 2, please read and agree to the
statement above by signing and dating it AND take all three pages of this form (Participant Questionnaire and the Physician’s Evaluation Form) to your physician for a medical evaluation. Participation in a diving course requires your physician’s approval.It is also recommended that you carry a physician signed copy of this form when you travel, as many dive operators may require it.
